Firefighter/Paramedic Job Application
- Authorized to work in the U.S.
- Must be 18 years of age at the time of the exam and 21 years of age, and under 35 years of age at time of hire, or as otherwise exempt by statute
- High school diploma (or its equivalent)
- Valid driver’s license
- Illinois Department of Public Health Licensed Paramedic or currently enrolled in a paramedic program (license due at time of hire)
- Color vision adequate to identify red, green, and yellow color. Visual acuity: 20/30 binocular each eye (corrected) with the ability to use a Self-Contained Breathing Apparatus through a corrected lens face piece or contact lens. Peripheral vision of 140 degrees or better
- Valid Candidate Physical Ability Test (CPAT) card and approved Ladder Climb Card required by time of hire

- Copy of valid driver’s license
- Copy of documentation for proof of legal authorization to work in the U.S. – may include county/state issued birth record, passport, naturalization papers, permanent resident card, work authorization card
- Copy of valid IDPH Paramedic License or proof of enrollment in a paramedic program
- Optional: Copy of valid CPAT and ladder climb
Virtual Testing Information:
Candidates must complete the written examination the week of June 30th. No evening or weekend sessions will be available, it is your responsibility to ensure you have the flexibility to test during this week. Visit https://iosolutions.com/candidateprep/ for more information on the virtual testing.
Interview Information:
Interviews will be conducted the week of July 14th and/or the week of July 21st, pending the number of eligible candidates. No reschedules will be permitted. It is your responsibility to ensure you have the flexibility to test during this timeframe. The interviews will evaluate merit criteria including Judgement, Teamwork, Interpersonal Skills, and Learning Orientation.
