Northlake, IL FPD Job Application 2023

Description

Minimum Requirements:

• $35.00 non-refundable application fee
• At least 21 years of age and under 35 years of age as of May 6, 2023, unless the individual falls under one of the exceptions to the statutory maximum hiring age detailed in 70 ILCS 705/16.06b(c)  Proof of date of birth is required
• No felony or disqualifying misdemeanor convictions
• High school diploma or equivalent (GED)
• A United States citizen at time of filing formal application or must meet requirements of the IL Human Rights Act for citizenship status 
• Valid driver’s license 
• Current Illinois or National Registry EMT-P (paramedic) Certification due at time of conditional offer. Applicants not currently in possession of an EMT-P license shall be allowed to complete the testing process by submitting proof of current enrollment in a State of Illinois IDPH approved paramedic program. Applicants must provide proof of enrollment in the program by submitting a letter of good standing from the organization sponsoring their enrollment or by the course administrator, on their letterhead.
• Successful completion of the CPAT and Ladder Climb certification conducted by a licensed agency due at time of conditional offer
• Must be of sound mental and physical health and meet all established standards
• Must be able to pass written test, medical examination and drug testing, polygraph examination, background investigation, and psychological examination

• Complete and Confirmed Online Application – available after purchase 
• Consent and Release Agreement (2 pages) – available after purchase 
• Copy of high school diploma or GED
• Copy of valid driver’s license
• Copy of birth record – can include: U.S. county/state issued birth certificate, U.S. passport, naturalization papers or permanent resident card
• Copy of valid IDPH or National Registry EMT-Paramedic license OR letter from organization sponsoring EMT-P enrollment
• Optional Copy of CPAT and Ladder Climb certifications issued  in the last 12 months (will be due at time of conditional offer)

Testing and Orientation Information:
Candidates must attend the mandatory orientation and written examination in the morning on Saturday, May 6, 2023.  Sign-in begins at 8:30 a.m.  Please arrive no later than 9:00 a.m. with a photo ID (driver’s license, state ID, military ID) to sign-in.  Test will be held at:
