Minimum Requirements:

• $35.00 non refundable application fee
• High school diploma/GED
• No felony convictions
• United States Citizen
• Valid driver’s license 
• At least 21 yeas of age and under 35 years of age as of September 14, 2022, unless the individual falls under one of the exceptions to the statutory maximum hiring age detailed in 70 ILCS 705/16.06b(c). Proof of date of birth is required 
• Must have proof of a valid CPAT and Ladder Climb certificate within 1 year of date of hire 

Required Documentation due August 17, 2022, at 4:00 p.m. (not accepted via email or fax):

• Online application (available after purchase)  
• Consent and release agreement (available after purchase)
• Copy of high school diploma
• Copy of valid driver’s license
• Copy of birth record – may include U.S. County/State issued birth record, U.S. Passport or Naturalization Papers 
• Copy of CPAT and Ladder Climb certificate(s) – optional 

Orientation and Testing:
Candidates must attend the mandatory orientation and written examination on Wednesday, September 14, 2022. Sign-in begins at 5:00 p.m. and closes at 5:30 p.m.

Testing will be held at:
Lake Villa Fire Station #3
1911 Grass Lake Rd.
Lindenhurst, IL

Candidates who required ADA accommodations must contact IOS Recruitment at least two weeks prior to the written exam date. All portions of the testing process are mandatory. Failure to attend and successfully complete any portion of the process will result in elimination from employment consideration.
